Indicate the date on which you are submitting your notice to the Canada Gazette Directorate.
Part Ⅰ or Part Ⅱ
Indicate in which part of the Canada Gazette your notice should appear. The content of Part Ⅰ includes proposed regulations, decisions of administrative boards and an assortment of government and private sector notices. Part Ⅱ includes approved regulations and other classes of statutory instruments, such as orders in council and proclamations.

Indicate the legal name of your department or agency.
Address
Provide the complete mailing address of the contact person.
Contact name
Provide the name of the person whom we should contact to acknowledge receipt of the notice and for questions about the content, if necessary, etc.

Indicate the telephone number of the contact name you provided.

Contact your departmental financial officer to obtain this code.
Department number
A three-digit number used to identify each department and agency. Contact your departmental administrative officer to obtain this number or go to www.tpsgc-pwgsc.gc.ca/recgen/pceaf-gwcoa/1011/3-eng.html#sec3.2.
IS organization code (IS Org)
Contact your departmental financial officer to obtain this code.
IS reference code (IS Ref)

Title of document submitted
Indicate clearly the title of your notice.
Published under the authority of (section of Act, Regulations, etc.)
Indicate the name of the act or regulations under which your notice must be published. Please note that all notices appearing in the Canada Gazette must be published in accordance with a particular law or regulations.

Please indicate whether you would like the email and Internet addresses in your notice to be activated in order that clients may access your Web site or email your department directly.
Extracts
The Canada Gazette Directorate can make arrangements to print additional copies of your notice as it appeared in the Canada Gazette. A minimum number of 100 copies is required. For information on the cost of printing extracts, please contact the Office Manager at 1-866-429-3885.

Indicate the expected date of publication. Refer to the Deadline Schedule for a list of publication dates and associated deadlines to ensure that your notice is submitted on time. Some deadlines vary due to statutory holidays.

Two copies of the English text and two copies of the French text, along with any artwork, should be submitted with each notice.

Text submitted on diskette, CD-ROM or USB Key
Save the English and French versions of your notice (Microsoft Word format only) as two separate files on diskette, CD-ROM or USB key that is not encrypted nor password protected. Notices saved on encrypted or password protected USB keys may not be processed and published on the required date. USB keys will be returned to the sender by mail.
